Let's start the Name Change Process
Step 1 - Affidavit
Make an affidavit concerning the change of your name at your local Notary. Your reason for changing your name must be stated in the affidavit. You may want to change your name, especially for women, after marriage. One of my friends wanted to start his own company. He was well-known in the area for his nickname. Note: This is a sample name change form. It is better to scan the original and keep an electronic copy on your external hard drive.
This affidavit is used to change your first or last names. You can also use the same to completely alter your name.
Step 2 - Newspaper publication
Sri ___________________ (old) has changed my name to ______________(new name) by an affidavit filed before the Notary Public at ______________ (place), on ___________. For all purposes, I will now be called _________________ (New name). ______________ (Name) & ________________ (Complete Postal Address).
Full Signature
Complete the form with all necessary details, and then publish it in your local paper.
Some tips
It can be published in any of the 2 local newspapers. You don't need to use FrontPage coverage. Even though you only need one copy, it is better to obtain ten copies of the newspaper in which your statement was published. Also, it is a lifetime issue so it is important to preserve it. It is better to keep both hard and scanned copies.
Step 3 - Gazette Notification
These are the steps you must take to get your name published in your state's Official Gazette.
1. Visit the local Government Press, or the department that mobilizes Gazette notifications.
2. 2.Obtain 2 copies of the "Deed Changing Surname Form". Fill out this form following the instructions.
3. Get the number or mobile number of the person in the department that can assist you with Gazette Notification.
Verification
The following documents should be sent to Secretary, Department of Commerce & Transport (Commerce), _________________(State).
- Let him know that you are writing to request his permission to change your name. (Click here for a sample letter).
- The "Deed Changing Surname Form”
- 2 True copies in the newspaper where your name change announcement was published.
- Attested copy the Name Change Affidavit for name change procedure in Delhi
- You will need to print a copy of the statement on plain paper for publication in your official Gazette. (Generally the same as your Newspaper Statement).
- A copy of your current identity, such as your Voter ID Card, PAN Card or Birth Certificate, and School or College Certificates, etc.
